Adding | Editing Question Forms

Navigation: Setup>>Res/Event Questions>>Questions Forms tab

Important Note: Questions forms are only available to be assigned to Flybook waivers or activities

If you would like to add or edit questions forms follow the steps below:

  1. Start by creating the questions you would like to include in the form.  
  2. Create a Question Group
    • This is where you can make questions Internal Only, Required, and/or Per Guest
  3. Go to the Questions Forms Tab
  4. From here you will be able to create forms by clicking the Create Form button
  5. Name your form and click Create
  6. Once created you will be able to attach a question group to it by clicking the name and then selecting the group
  7. Once selected click Update
  8. This form will now be available to be applied to either an activity or a Flybook waiver

Question forms can be added to waivers or to activities to use as a Post Use form.  

To assign a Question Form to a waiver, go to Setup > Waivers.  You will have the option to 'Use in waiver question form' or to assign it as 'Post Signing Waiver Questions Per Participant'.  

To assign to an activity as a Post Use Form, go to the Activities calendar and click into a scheduled activity.  Go to the Questions tab.  Select the form in the dropdown and click 'Add Form'.
