Navigation: Setup>>Res/Event Questions>>Questions Forms tab
Important Note: Questions forms are only available to be assigned to Flybook waivers or activities
If you would like to add or edit questions forms follow the steps below:
- Start by creating the questions you would like to include in the form.
- Create a Question Group
- This is where you can make questions Internal Only, Required, and/or Per Guest
- Go to the Questions Forms Tab
- From here you will be able to create forms by clicking the Create Form button
- Name your form and click Create
- Once created you will be able to attach a question group to it by clicking the name and then selecting the group
- Once selected click Update
- This form will now be available to be applied to either an activity or a Flybook waiver
Question forms can be added to waivers or to activities to use as a Post Use form.
To assign a Question Form to a waiver, go to Setup > Waivers. You will have the option to 'Use in waiver question form' or to assign it as 'Post Signing Waiver Questions Per Participant'.
To assign to an activity as a Post Use Form, go to the Activities calendar and click into a scheduled activity. Go to the Questions tab. Select the form in the dropdown and click 'Add Form'.
